Same issue for me since 3.01 update:
- Login to game client, load character.
- Load Minion launcher.
- Attach to ffxiv client.
- The ffxiv client terminates / crashes.

CCleaner( what is CCleaner if its malware tool REMOVE IT)
No recording software.
No prior problems with installation.( WE DONT CARE ABOUT NO PRIOR PROBLEMS READ THE STICKY)
Bot running as admin.
Using the attach button.
No bot addons.

You sir have killed 3 puppies maybe 4..... You need to re read the set up instalation guide found HERE and do what it says dont choose to do just part of it.
