If you have never reinstalled it since beta, you will need to also delete the configuration files inside of "My Documents > My Games > FFXIV".
Inside of that folder you will find several folders with .DAT files in them like HOTBAR.DAT, which you may want to back up so you can recover your UI settings.
I'm surprised you haven't ever had issues updating the game without doing a complete removal in this manner after the beta, it was a pretty widespread problem.
